Staff Registrar Scheme (SRS)
The Staff Registrar Scheme (SRS) Diplomas have been implemented as part of the collaboration with the Ministry of Health, Singapore (MOH) to enable our public sector healthcare institutions to employ doctors who are not specialists but are able to provide a middle level of service equivalent to that provided by a Staff Registrar or Registrar. Conducted as a structured diploma course with on-the-job training, the SRS also accommodates foreign doctors whose specialist degrees are not recognised by the Specialist Accreditation Board (SAB) and whose medical schools are not recognised by the Singapore Medical Council (SMC). The SRS Diplomas are under the purview of the Standing Committee on Postgraduate Education, Academy of Medicine, Singapore.

Programme

Advanced Diploma in Pathology

Course Duration

2 years

Course Objectives

The aim of the programme is to enable the trainee to attain the following General Competencies:

  • Understand the local working environment and resources available;
  • Competence in specimen handling and diagnostic pathology reporting;
  • Effective communication skills and interpersonal skills

Exam format

All trainees are required to undergo an exit examination at the end of their training period.

Rehabilitation Medicine

Course Duration

2 years

Course Objectives

The aim of the programme is to enable the trainee to acquire the skills and knowledge in the following:

  • Skills and knowledge in the evaluation and management of patients in Rehabilitation Medicine
  • Management of common medical complications that occur in rehabilitation patients
  • Effective communication skills and professionalism in rehabilitation care
  • Awareness and knowledge of the social support system in the local context

Exam format

All trainees are required to undergo a yearly examination. Trainees must pass this examination before they can continue to the next training year.

Programme

Neonatology

Course Duration

2 years

Course Objectives

The aim of the programme is to enable the trainee to acquire the skills and knowledge in the following:

  • Knowledge in perinatal counselling, neonatel resuscitation, newborn screening, routine care of the newborn baby, neonatel intensive care as well as special care of sick neonates.
  • Develop competence in a wide spectrum of basic science knowledge, clinical and procedural skills while displaying professional skills.

Exam format

All trainees are required to undergo a yearly examination. Trainees must pass this examination before they can continue to the next training year.

Programme

Medical Oncology in Hospital

Course Duration

3 years

Course Objectives

The aim of the programme is to enable the trainee to acquire the skills and knowledge in the following:

  • History taking and examination techniques in the cancer patient.
  • Managing acute oncology emergencies commonly encountered in hospital practice.
  • Diagnosing and treating common cancers.
  • Principles of pharmacology in relation to chemotherapy administration.
  • Chemotherapy administration including indications, dose modifications, side effects and their management.
  • Medical procedures commonly carried out in cancer patients (lumbar puncture, inthrathecal administration, bone marrow aspiration and trephine biopsy, abdominal paracentesis, pleural tap, insertion of percutaneous long line).
  • Effective interpersonal and communication skills pertaining to the cancer patient, including breaking bad news, discussing resuscitation status, withholding or withdrawing therapy, informed consent, educating patients, and working within multidisciplinary teams as members.
  • The basics of evidence based medicine and its application to clinical practice.

Exam format

All trainees are required to undergo a yearly examination. Trainees must pass this examination before they can continue to the next training year.

Programme

Clinical Haematology (Adult)

Course Duration

2 years

Course Objectives

The aim of the programme is to enable the trainee to acquire the skills and knowledge in the following:

  • Knowledge and understanding of the practice of Clinical Haematology
  • Skills in the diagnostic techniques required in the practice of haematology eg Bone marrow aspirates and trephines, administration of chemotherapy
  • Management of hematologic emergencies e.g. leucostasis syndromes, tumor lysis syndromes, extravasation of vesicant chemotherapeutic agents
  • Knowledge of specialist areas within haematology such as blood transfusion, bone marrow transplantation, sufficient for general haematology practice
  • Acquire communication skills required for practice of clinical haematology
  • Understand the tools of research in improvements in clinical practice and the need for continuing professional development for the maintenance of standards of practice

Exam format

All trainees are required to undergo an exit assessment conducted by the Chapter at the end of the training programme. A panel of 3 specialists, nominated by the Academy, will review the training/education portfolio, log book and conduct the assessment.

Programme

Emergency Medicine

Course Duration

2 years

Course Objectives

  • Acquire the knowledge needed to manage the wide variety of clinical problems commonly encountered in the Emergency department
  • Demonstrate the procedural skills integral to Emergency department work
  • Demonstrate patient safety awareness, sound clinical reasoning and decision making through interactive tutorials and case studies
  • Acquire communication skills and gain an understanding of ethical issues unique to emergency medicine through seminars, interactive tutorials and case studies
  • Demonstrate basic ability to access, review and apply pertinent medical and scientific literature through the completion of a case report and focused literature review

Exam format

For entry into the Final Assessment (MCQ and PACES), the trainee must have:

  • 90% attendance for all practicum / hands-on sessions of the Diploma Course
  • 80% attendance for all other teaching sessions of the Diploma Course
  • completed all semester multiple choice questions (MCQ) tests and semester Practical Assessment of Clinical Examination Skills (PACES)
  • completed all online assessments
  • completed the case report and focused literature review
  • Valid certificates as Basic Cardiac Life Support (BCLS) provider and Advanced Cardiac Life Support (ACLS) provider. These certificates must be issued by a Singapore National Resuscitation Council accredited training centre

Paediatrics & Child Health

Course Duration

2 years

Course Objectives

  • To provide the additional number of staff to develop and sustain services in the Paediatrics department
  • Provide an alternative career track for local Medical Officers (MOs) who are not keen on specialist training and would like to have a long-term career in Paediatrics
  • Provide structured training for foreign-trained doctors who come for training in Paediatrics

Exam format

Trainees are required to partake in the following exams:

  • Oral Exam at the end of Year 1 (Part 1)
  • The candidate is expected to complete Part 1 of the training by the end of Year 1.
  • Certification of Completion of Training (Oral Exam) at the end of Year 2 (Part 2)
  • MCQs Exit Examination at the end of Year 2

Programme

Infectious Diseases

Course Duration

2 years

Course Objectives

  • To obtain clinical knowledge in Infectious Diseases and HIV medicine
  • To demonstrate clinical competency in the care of patients

Exam format

There will be a practical examination consisting of short cases at the end of the first academic year. Final examination will also be in the same format at the end of the second academic year. To be eligible for the final examination, the trainee must have:

  • Completed all requisite clinical rotations (inpatient care at the Communicable Disease Centre, Tan Tock Seng Hospital and the Department of Infectious Diseases, Tan Tock Seng Hospital for 18 to 24 months and 50 infectious diseases consultations every 6 months)
  • Attended at least 80% of the teaching sessions
  • Passed all 4 clinical assessment modules

Programme

Nuclear Medicine

Course Duration

2 years

Course Objectives

  • To address manpower and service requirements in public sector hospitals at Registrar or Senior Registrar level
  • To provide the additional number of staff to develop and sustain specialty services in the department without inadvertently escalating the number of specialists in the process
  • Allows departments to retain and train suitable non-specialist doctors {e.g. doctors on temporary registration (service) who can provide continuity of care in the transition between MO postings

Exam format

Trainees will be required to undergo exit assessment conducted by the Chapter at the end of the Diploma course. At least 2 specialists, nominated by the Academy will review the education/training portfolio and conduct the necessary assessment.

Programme

Neurosurgery

Course Duration

3 years

Course Objectives

The Diploma in Neurosurgery programme is designed to train a potential Staff Registrar who will attain knowledge and technical skills in diagnosing, treating, monitoring and managing patients with neurological problems, disorders of nervous system, emergency neurotrauma and multiple system trauma. Trainees will acquire competencies in clinical practices and surgical procedures, good clinical judgements and decision making, effective communication and ethical knowledge associated with Neurosurgery. At the end of the programme, trainees will also gain knowledge in ethical expectations that impact on medico-legal issues and make aware of current legal aspects of informed consent and confidentiality and be accountable for their patients.

Exam format

The Chapter of Neurosurgeons, College of Surgeons, Singapore shall conduct a yearly interview with the trainees. Trainees are expected to pass the yearly interviews throughout the three years of training.

Final examination format will include multiple choice questions, written essays and clinical examinations.

Programme

Geriatric Medicine

Course Duration

2 years

Course Objectives

The objective of the training programme is to equip trainees with the skills and knowledge of evaluating and managing older patients in the hospital and intermediate and long term care (ILTC) settings. The structured programme and the close supervision of trainees will ensure the development of a safe and competent potential Staff Registrar.

Exam format

Trainees will be required to sit for 6 monthly interviews throughout the Diploma course. At the end of the first year, the examination will be organised by the Division of Graduate Medical Studies of Yong Loo Lin School of Medicine, NUS. The examination format consists of:

a) Written Examination

b) Practical Examination which includes:

i. Short Case examination

ii. Long Case examination

iii. Viva examination.

They are required to complete and pass the first year interviews and exam before proceeding to the second year of training.

At the end of the second year, the exit examination will be organized by Chapter of Geriatricians and will comprise of:

a) Ward Round assessment

b) Viva station

Programme

Orthopaedic Surgery

Course Duration

3 years

Course Objectives

  • T o develop and sustain a high quality specialty services in Orthopaedic Surgery by retaining and training temporarily-registered (service), conditionally-registered doctors as well as fully-registered medical practitioners who have an interest in Orthopaedic Surgery and wish to continue a career in Orthopaedic Surgery but who do not qualify or do not wish to be a Specialist.
  • To ensure the trainee has an adequate theoretical grounding in Orthopaedic Surgery so to enable them to work independently at the level of a Staff Registrar.
  • To ensure the trainee is able to gain practical and surgical skills enabling them to manage patients at the Staff Registrar level.

Exam format

Trainees are required to take the Orthopaedic In-Training Examination (OITE) annually.

At the end of the 3 year training period, trainees will be required to undergo an exit exam conducted by the Chapter of Orthopaedic Surgeons.

Cardiothoracic Surgery

Course Duration

3 years

Course Objectives

  • Provide an alternative career track for local Medical Officers (MOs) or Clinical Associates (CAs) who are not keen to go through specialist training and would like to have a long-term career in Cardiothoracic Surgery
  • Provide a structured training for eligible foreign-trained doctors who wish to receive advanced training in Cardiothoracic Surgery
  • Provide an avenue for institutions to retain good, temporarily-registered (service) doctors, by providing training to enable them to assist in cardiac operations and manage the cardiothoracic intensive care unit.

Exam format

Trainees will be assessed in the following manner:

End of Year 1 - Viva

End of Year 2 - Appraisal by supervisor & Viva

End of Year 3 - Appraisal by supervisor & Final exam consisting of Viva and Presentation of clinical case studies (both long and short cases)

Cardiology

Course Duration

2 years

Course Objectives

  • To provide opportunity for development of staff registrar based on candidates for those already be qualified in Internal Medicine overseas but do not satisfy our national professional requirements for registration as specialists in Cardiology.
  • Allows departments to retain and train suitable foreign-trained doctors {eg doctors on Temporary Registration (service)}to the competency level of a Staff Registrar who can provide registrar level service work and who can provide continuity of care in the transition between MO postings.

Exam format

All trainees will be required to undergo an exit assessment conducted by the Chapter of Cardiologists at the end of the training period. To obtain the diploma, the candidate must successfully sit through a clinical cardiology MCQ assessment and a set of data interpretation questions.

Palliative Medicine

Course Duration

2 years

Course Objectives

To enable the trainee to:

  • acquire the knowledge needed to manage the wide variety of clinical problems commonly encountered in the terminally ill
  • acquire the skills and knowledge needed to manage the wide variety of psychosocial and spiritual problems encountered in the terminally ill and their families / loved ones
  • acquire communication skills essential in the care of the terminally ill patients;
  • gain an understanding of ethical issues unique to caring for the terminally ill through interactive tutorials and case studies
  • demonstrate sound clinical reasoning and decision making through interactive tutorials and case studies
  • assess and review pertinent medical and scientific literature through the completion of case reports and focused literature review, and apply it to practice;
  • understand the various settings in which palliative care is delivered, thereby ensuring continuity of care for the terminally ill

Exam format

For entry into the Final Assessment (MCQ and PACES) for the Diploma course, trainees must:

  • have attended 80% of all teaching sessions of the Diploma Course
  • Have 80% attendance for all other teaching sessions of the Diploma Course
  • Have completed all semester multiple choice questions (MCQ) tests and semester Practical Assessment of Clinical Examination Skills (PACES)
  • have completed the case reports and focused literature review.

Hand Surgery

Internal Medicine (with focus on Hospital Medicine)

Course Duration

2 years

Course Objectives

  • To acquire core knowledge of medical conditions that are frequently seen in hospitalised patients in the General Medicine department
  • To attain proficiency in history taking and examination techniques in the hospitalised medical patient
  • To attain proficiency in medical procedures (lumbar puncture, bone marrow aspiration, abdominal paracentesis, pleural taps, joint aspiration)
  • To attain clinical competency in managing acute medical emergencies commonly encountered in hospital practice
  • To attain clinical competency in managing a wide variety of chronic and complex medical conditions and their complications
  • To learn effective interpersonal and communication skills including difficult issues such as resuscitation status, futility, withholding or withdrawing therapy, informed consent, educating patients, working within multidisciplinary teams as members
  • To learn the basics of evidence based medicine and its application in day to day clinical practice
  • To learn effective skills which facilitate safe and seamless transitions of care

Exam format

Bi-Monthly Mini-CEx (Mini-Clinical Examination).

Bi-Monthly patient care competency based evaluation for each rotation.

A final examination will be conducted at the end of the course and will consist of the following components:

  • MCQ test
  • Short questions on data interpretation (including ECG and radiology)
  • Clinical examination consisting of one long case and 2 short cases

Obstetrics & Gynaecology

Course Duration

4 years

Course Objectives

  • To equip trainees with well-rounded and supervised training in clinical Obstetrics and Gynaecology (O&G) under the supervision of a senior physician
  • To imbue trainees with an appreciation of the ethics of the O&G specialty
  • To train potential Staff Registrar who are well qualified and competent in providing a high standard of care in the O&G specialty that matches those of the trainees in other countries at a corresponding stage of training

Exam format

Trainees are continuously assessed through practical and written examinations every year.
